PRIVATE REFERRALS – INSURANCE CLAIM FORMS

We receive many such forms every week.  In order to ensure these forms are completed correctly, please provide your doctor with the following details and please note that we cannot complete these forms unless you have been referred by your GP.

1. Date of referral
2. Name of specialist to whom referred
3. Dates of hospital admission (if appropriate)

N.B.  We need at least 48 hours for completion of these forms.
