We are happy to register new patients who live within the practice area.
If you would like to register with the surgery please visit the surgery and speak to one of our receptionists.
If you have a medical card from your previous practice you can bring it with you.
Please print out and fill in our patient questionnaire and bring this with you. You can find a copy of the questionnaire by clicking here.
You must bring proof of identification, such as a valid passport, driving licence or identity card. You must also bring a document that gives proof of residence such as a utility bill (e.g. gas, electricity or telephone), Council Tax bill or a tenancy agreement. |
