As health and care professionals we have a responsibility to keep health care records accurate and up to date.

However, mistakes can occasionally happen, and we sincerely apologise for this.  As a patient you have the right to request that your records are rectified.

You can use this form to tell us if any of your demographic information is incorrect (for example, your date of birth, age and ethnicity)   

We can only amend factual information. We cannot delete medical entries but we can make a retrospective change.  

If you have seen something in your records where your interpretation differs from that of a health professional, it is unlikely that we can alter your records. Please do not use this form in such an instance.

There are times, for example, where a working diagnosis, which at the time of entry was clinically possible, has been ruled out later and a different confirmed diagnosis made.

This does not mean that your records are inaccurate, we cannot alter your medical record in such instances. However, if you have a concern you would like to discuss please use our contact us form instead, fully explaining your concerns and we will be in touch.
